Dr Martina Hens retired from The Swan Practice on 31st December 2021

We would like to share that Dr Martina Hens has retired from The Swan Practice on 31st December 2021. Please see below message from Martina, we will all miss her very much. 

 

Dear All,

I would like to let you know that I will be retiring from The Swan Practice on 1st of January 2022.

It has been a privilege to work as a GP in and around Buckingham in the past nearly 25 years.

I have met and got to know a lot of wonderful people both patients and colleagues and I am left with many great memories of my time initially at Verney Close Family Practice and after the merger, at The Swan Practice.

However, I feel the time has come to move on and I am confident that I leave ‘my’ patients in the capable hands of all the team at the practice.

Thank you to all staff, patients, members of the primary health care team and all the others I worked with and who helped doing the best for our patients.

I wish you all the best for the future, look after yourselves and take care, and hopefully I will see many of you around in Buckingham in the future.

Community Pharmacy Consultation Service

** Service temporarily suspended **

The Swan Practice is pleased to be adopting a new NHS service to improve access to a clinician for our patients. Community pharmacists are highly trained professionals who can help with several conditions that our patients call in with. The NHS has created a system where community pharmacists can assist with primary care capacity. This is additional to our current service, and entirely optional for patients.

 All four of the pharmacies in Buckingham have signed up for this. The service has been used elsewhere since 2019. For certain specified conditions, when you call in, your care navigator may offer you a referral to the pharmacy as an alternative to a triage call from one of our clinicians. If you agree, with your consent, information about your problem will be passed electronically to the pharmacy of your choice. The pharmacist will then contact you promptly. They can discuss your problem over the telephone or face-to-face in their consulting room. Many conditions can be treated with over-the-counter medication rather than requiring a prescription.

If the pharmacist decides your problem needs something more than they can provide or prescribe, they will refer you back to us on the same day and one of the Swan clinicians will contact you.

We hope that this service will improve prompt access for our patients to the appropriate clinician, and we welcome any feedback you may have on this.

Patient Access - Memorable Word Security Measure

We are aware that Patient Access has now introduced a 'memorable' word as an additional security measure when logging into your patient access account.  This has generated a large volume of calls to the practice .  The patient access support team are the people best placed to help with these queries and can be contacted by going to www.patientaccess.com.

Did you  know that you can now reset your password or your memorable word by clicking on 'forgotton password' or forgotten 'memorable word'

 


 

Wearing Face Masks

As you are already aware wearing a face mask will become mandatory in certain situations from the 24th July 2020. As part of the government’s guidance some people may be exempt for health, age or equality reasons.

We are starting to have some questions from patients about whether they need letters from us regarding exemption from wearing a face mask.

The good news is that you don’t!

GPs are not able to provide letters of support if you do not fall in to these exemption groups.

People needing to travel on public transport in London may wish to print out a self-exemption card, approved by TFL: https://tfl.gov.uk/campaign/face-coverings

If you are travelling abroad you will need to adopt the requirements of the country you are visiting. If you require evidence of a medical condition that would mean you were exempt in the UK when you travel abroad then we may be able to provide you with a private certificate

 


 

Nurse Olivia is on-hand 24/7- Download the new app Ask NHS

The app is free to download on any mobile device and provides patients with a symptom checker, self-care advice and access to book the most appropriate GP appointment or direct you to NHS 111, if required.

Using the app can help you take more control of your own health.  A virtual healthcare assistant, Olivia, will ask you a series of questions (approximately 15) to assess your symptoms in a clinically safe and NICE accredited way. Olivia will then recommend the most appropriate care for you which might be self-care advice via NHS Choices, appropriate GP appointment, GP call back or direction to urgent or emergency care.

For patients who need an appointment, the app can prioritise those patients into the most appropriate time slot based on their needs.  You’ll be able to directly book an appointment without having to call the practice.

PLEASE NOTE If you have an on-going problem which is being looked after by the GP, then please call the practice for an appointment which will provide continuity of your care.

Electronic Prescribing Service (EPS)

Electronic Prescribing Service (EPS) enables prescribers - such as GPs, Practice Nurses and Pharmacists - to send prescriptions electronically to a pharmacy of the patient's choice. This makes the prescribing and dispensing process more efficient and convenient for both patients and staff.

Please speak to your preferred pharmacy about setting up a nominated pharmacy if you wish to take up this service. If you already have a nominated pharmacy, your prescriptions will automatically be sent electronically.

The process for patients who have their prescription dispensed at the practice remains unchanged.

If you don't want your prescription to be sent electronically please inform your GP when you next need a prescription.

If you want to change or cancel your pharmacy nomination speak to dispensary. Please tell them before your next prescription is due or your prescription may be sent to the wrong place.

 


 

Chain SMS (accuRx) Text Messaging 

We are now using Chain SMS (accuRx) to message patients.

Chain SMS is.........

A system which allows us to easily send text messages to patients. It means we can be much more proactive about some communications, messaging you quickly and securely, so that you are not waiting around to hear from us.

Some examples of how we might use it in the future....

  • Sending you an appointment
  • Reminders or notifications (e.g. prescription is ready)
  • Responding to simple queries (e.g. if you had a quick question about your medication)
  • Letting you know we tried to call
  • Sending you advice at the end of a consultation

The practice name will always be at the bottom of the message.

You won't be able to reply to the message. 

We will routinely be asking you for your consent to receive text messaging

Please update your records at Reception, especially if you share a mobile number with another patient -e.g. parent, partner or carer

Please help us to help you by keeping your mobile number up to date.
